Refer regardless of PCP access. IF PCP appointment can be established quickly, CCC can cancel the referral.

If patients are out-of-network, they will likely receive a bill, and typically should be instructed to go in-network instead.

If patients are out-of-county and uninsured, these patients will receive a bill but can follow-up.

If patient does not have a working phone, give them instructions in their paperwork for how to walk to CCC during daytime hours to schedule an appointment. (dot phrase to come!)

newly diagnosed malignancy that does not meet admission criteria

coordination of colonoscopy or endoscopy

wound care (wound vac, diabetic foot infections, vascular wounds, ostomy, fistula) - specific staffing by wound-certified NPs
