    • Rapid follow-up (typically within 5 days)​

    • Refer regardless of PCP access. IF PCP appointment can be established quickly, CCC can cancel the referral.

    • If patients are out-of-network, they will likely receive a bill, and typically should be instructed to go in-network instead.

    • If patients are out-of-county and uninsured, these patients will receive a bill but can follow-up. 

    • If patient does not have a working phone, give them instructions in their paperwork for how to walk to CCC during daytime hours to schedule an appointment. (dot phrase to come!)

      • Take elevator down one floor. When you step out of the elevator, turn left.

    • Examples of what to refer:

      • poorly controlled DM, HTN​

      • chronic conditions: CHF, COPD

      • newly diagnosed malignancy that does not meet admission criteria

      • coordination of colonoscopy or endoscopy

      • wound care (wound vac, diabetic foot infections, vascular wounds, ostomy, fistula) - specific staffing by wound-certified NPs
