  • Eligibility: 18+ yo, no PCP, and released from jail/prison within last 12 months

Denver jail: Contact

For patients being discharged to Denver Jail, always communicate the follow-up plan in the AVS.

For complex discharge plans, you can also:
  • Call 720-337-0167, 720-337-0168, and/or
  • Send an epic staff message to providers via "DH CCMF" pool (see below)
    • Provide patient name, DOB, MRN, and follow-up needs

Denver Jail: Trauma protocol

Trauma protocol at the jail is Q4H VS and neuro checks for 24H

Trauma patient that need serial exams otherwise (eg. serial abdominal exams) should NOT be sent to jail until those are completed.

Psychiatric Complaints

Patients discharging to the Denver City/County jail receive can psychiatric care there and cannot be evaluated by PES.

Patients discharging to any other correctional facility should have any psychiatric complaint evaluated by PES.
